IMSI (Intracytoplasmic morphologically selected sperm injection)

IMSI, one of the auxiliary techniques used to increase the success rate of IVF treatment, is a sperm selection method used in intracytoplasmic sperm injection (ICSI).1

In the IMSI technique, which is used to increase the chance of creating quality embryos by selecting the sperms which have the proper morphology, it is possible to examine the sperms by enlarging them by approximately 8000 times. The fact that sperms, which can be enlarged approximately 400 times under classical laboratory conditions, can be examined in such high clarity allow us to be able to select the ones with the best head, body and tail structures. 

In patient groups where the morphology of the sperms is below the optimal levels, the method used to select the best sperms is usually performed simultaneously with preimplantation genetic diagnosis and aneuploidy test (numerical chromosomal anomaly screening). Research on IMSI is ongoing. As a result of the studies carried out so far, it is not possible to make definitive statements about the mechanism of positive effect of IMSI on pregnancy and live birth rates compared to traditional ICSI. 

In Which Cases Can IMSI be Performed?

  • Recurrent IVF failures,
  • When fertilization cannot be achieved with the ICSI technique,
  • If quality embryos cannot be obtained,
  • In cases of infertility of unknown causes and
  • In the absence of optimal values in sperm morphology, IMSI may be considered as an option. 

IMSI is an additional technique that is performed only during the selection of sperm. It does not change IVF stages. The sperm, which is selected with IMSI is mechanically transfered into the egg via ICSI method. From this perspective, it is possible to define IMSI as an advanced version of the ICSI method in terms of the study of sperm morphology.

It is Possible to Select Better Quality Sperms using Various Sperm Selection Methods

Considering the treatments that increase the success rate of IVF treatment, it can be stated that there are various options for increasing embryo quality and promoting embryo transfer. Sperm selection techniques stand out as options that have become increasingly important in recent years and positively affect the success of the treatment process.

Although successful results can be obtained with the traditional ICSI technique, and the merging of sperm and egg can be facilitated, new methods of selecting the highest quality sperm for fertilization can increase the chances of success. IMSI is one of these methods. Within the semen analysis performed, in line with Kruger's criteria, the morphological analysis of the sperms is performed. When the results obtained show that there is a morphological problem with the sperm cells, or more precisely, that there are more deformed sperms
